Along the anteromedial ankle, the tendon of tibialis anterior crosses anterior to the tibiotalar joint to insert on the medial cuneiform and base of the first metatarsal, while extensor hallucis longus runs more centrally to the dorsal aspect of the distal phalanx of the great toe. Superficial veins and cutaneous landmarks are typically apparent on the dorsum, with the skin and subcutaneous tissue draped over the midfoot arch and the first ray. Clear medial relationships dominate. This medial dorsal perspective matters when teaching surface anatomy and tendon topography for the anteromedial ankle, where clinicians palpate tibialis anterior and extensor hallucis longus to assess dorsiflexion strength and gait dysfunction such as foot drop.
