Description
Centered on the posterior thoracic wall of an adult woman, the triangle of auscultation is delineated between the medial border of the scapula laterally, the trapezius muscle medially and superiorly, and the latissimus dorsi inferiorly. The floor is formed by the rhomboid major overlying the intercostal spaces, where the muscular covering thins compared with adjacent regions of the back. Superficial landmarks such as the spinous processes, the scapular spine, and the inferior angle of the scapula help orient the triangle in anatomical position. Boundaries are shown in clear relation to the scapulothoracic contour. Clinically, this small gap in muscular coverage is a practical window for posterior lung auscultation, most often over the upper portion of the lower lobe and adjacent posterior segments when the scapula is protracted. Ask the patient to fold the arms across the chest and lean forward, and the scapula moves laterally, widening the triangle and bringing the thoracic wall closer to the stethoscope. This is where subtle crackles from early pneumonia, fine inspiratory rales in interstitial lung disease, or localized decreased breath sounds from a posterior pleural effusion can be easier to appreciate than through the thicker paraspinal and scapular musculature. Teaching teams use this view when coaching surface anatomy and physical examination sequencing in undergraduate gross anatomy, clinical skills, nursing assessment, and respiratory therapy curricula. It also suits atlas plates, OSCE station materials, and patient-facing diagrams that explain why clinicians reposition the shoulders during back auscultation and where they are listening relative to the scapula and ribs.
