Seen in lateral profile, the female hindfoot is organized around the calcaneus (calcaneum), whose posterior calcaneal tuberosity forms the palpable heel and sits inferior to the distal tibia and fibula. Superior and posterior, the calcaneal (Achilles) tendon courses to its insertion on the posterior calcaneus, while the plantar aspect transitions anteriorly into the plantar fascia arising from the medial process of the calcaneal tubercle. Superficial to bone, the plantar heel fat pad thickens beneath the tuberosity and is bounded by glabrous plantar skin, with the lateral malleolus and adjacent retromalleolar contour serving as proximal landmarks for orientation. Clinically, this view maps directly to the common pain generators at the posterior and plantar calcaneus: insertional Achilles tendinopathy, retrocalcaneal bursitis, and Haglund deformity at the posterosuperior calcaneal prominence, versus plantar fasciitis and calcaneal spur formation at the medial plantar tubercle. Heel pad syndrome also reads well from a lateral silhouette because the relationship between the calcaneal tuberosity and its fibroelastic fat compartments explains why atrophy or displacement produces focal tenderness during weight bearing.
