Description
Running from the calcaneal tuberosity posteriorly to the head of the fifth metatarsal anteriorly, the lateral (fibular) border of the male foot is defined by the lateral aspect of the heel and the outer margin of the midfoot and forefoot. Along this edge, the cuboid sits anterior to the calcaneus and proximal to the bases of the fourth and fifth metatarsals, while the shaft and tuberosity of the fifth metatarsal form the most prominent lateral bony contour. Superficial landmarks expected in this region include the lateral malleolus superiorly at the ankle and the course of the fibularis (peroneus) longus and brevis tendons as they pass posterior to the lateral malleolus toward the lateral midfoot. This is the foot’s lateral column in profile. Clinically, the lateral border is where many exam findings start: focal tenderness at the base of the fifth metatarsal raises concern for an avulsion fracture at the tuberosity (often with inversion injury and traction from fibularis brevis), while pain more distal along the proximal diaphysis suggests a Jones fracture with different healing risk and management. The peroneal tendons are assessed here for subluxation behind the lateral malleolus, and the calcaneocuboid region is a key site in midfoot sprains and cuboid syndrome. Shoe wear patterns along the outer edge also reflect hindfoot varus or a supinated gait, a practical link between surface form and biomechanics.
