Description
Centered in the male midfoot, the intermediate cuneiform sits distal to the navicular and proximal to the base of the second metatarsal, forming a keystone articulation in the medial column. Medial and lateral to it lie the medial cuneiform and lateral cuneiform, aligned proximodistally between the navicular and the first and third metatarsal bases. Lateral to the cuneiform complex, the cuboid occupies the lateral column, positioned distal to the calcaneus and proximal to the fourth and fifth metatarsals. The metatarsus is shown immediately distal to the tarsal bones, with the metatarsal bases flaring proximally to meet their respective tarsometatarsal joints. Clear depiction of the intermediate cuneiform and its relationship to the second metatarsal matters because this joint line becomes a reference when teaching the Lisfranc complex, where subtle diastasis between the medial cuneiform and second metatarsal base can signal an unstable injury.
