Description
Centered on the junction between the lower lip (labium inferius) and the soft-tissue chin (mentum), the mentolabial (labiomental) sulcus is rendered as a distinct transverse depression separating the inferior vermilion and cutaneous lower lip from the convexity of the mental region. Superior to the fold lies the labial tubercle and the labiomental portion of the orbicularis oris, while inferior and deep to the crease the mentalis muscle shapes the chin pad and pulls the lower lip and skin superiorly. Laterally, the sulcus tapers toward the oral commissure, blending with the perioral cutaneous topography and the marionette line region. Facial analysis often hinges on this landmark because its depth and angulation reflect the balance between dentoalveolar support, mandibular projection, and mentalis tone, a triad that becomes obvious in profile and three-quarter assessment for orthodontics, genioplasty, and lower-face feminization planning. In aesthetic practice, the labiomental fold is a common target for hyaluronic acid filler placement or neuromodulator treatment of a hyperactive mentalis, but overcorrection can blunt the sulcus and produce an unnatural lower lip-chin continuum. Small errors show immediately. Plastic surgery texts, dental and orthodontic curricula, and maxillofacial operative planning materials use this view to teach surface anatomy, aging changes, and soft-tissue response to skeletal movement, as well as to illustrate injection vectors and safe zones around the inferior labial region.
