Description

Seen from the medial aspect of the male ankle, the distal tibia forms the medial malleolus, which descends inferiorly to overhang the talus at the talocrural (ankle) joint line. Immediately inferior and slightly anterior sits the talar head and neck, transitioning distally toward the medial longitudinal arch where the navicular tuberosity creates a palpable prominence. Posterior to the malleolus, the calcaneus and the contour of the Achilles (calcaneal) tendon outline the hindfoot, while the soft tissue groove between malleolus and calcaneus corresponds to the tarsal tunnel region. Medial surface topography matters because this side concentrates clinical landmarks you palpate first when localizing pain or planning exposure around the ankle. Tenderness at the navicular tuberosity often tracks with posterior tibial tendon dysfunction and adult acquired flatfoot, while focal pain over the medial malleolus raises concern for deltoid ligament injury or an avulsion fracture after an eversion mechanism. The tarsal tunnel corridor, posterior to the malleolus and deep to the flexor retinaculum, is a frequent site of tibial nerve compression and is approached with careful respect for the posterior tibial artery and accompanying veins. Palpation guides the diagnosis.
