Description
Oriented to the adult female groin, the inguinal ligament runs obliquely from the anterior superior iliac spine to the pubic tubercle, forming the inferior border of the anterior abdominal wall and the superior boundary of the femoral triangle. Superior to the ligament, the inguinal canal is defined by the aponeurosis of external oblique anteriorly, transversalis fascia posteriorly, and the arching fibers of internal oblique and transversus abdominis as a roof, with the superficial inguinal ring positioned just superolateral to the pubic crest. In the female, the round ligament of the uterus traverses the canal toward the labium majus, while the inferior epigastric vessels lie posterolateral to the deep inguinal ring, a key landmark when distinguishing indirect from direct hernia pathways. Medial to the femoral vein, the femoral canal and its septum sit below the inguinal ligament, bounded by the lacunar ligament medially and the pectineal ligament posteriorly. Small but consequential. Groin anatomy in women matters because the differential shifts: femoral hernias are proportionally more common than in men and often present with incarceration below and lateral to the pubic tubercle, easily confused with inguinal pathology on exam. Clear relationships among the inguinal ligament, lacunar ligament, femoral vein, and the round ligament guide open and laparoscopic hernia repair, and they also explain why lymphatic spread from the vulva and lower abdominal wall reaches the superficial inguinal lymph nodes.
