Seen from the lateral aspect, the inferior palpebra is defined by the cutaneous lower lid anteriorly and the palpebral conjunctiva lining its posterior surface, with the tarsus (inferior tarsal plate) forming a firm core just deep to the lid margin. The eyelashes arise along the anterior lamella near the gray line, while the meibomian gland orifices align along the posterior lid margin adjacent to the lacrimal lake. At the lateral canthus, the upper and lower lids converge and anchor to the lateral orbital rim via the lateral palpebral raphe and lateral canthal tendon, placing the tarsal plate medial to that fixation point. Lateral lower eyelid anatomy is a constant reference in oculoplastic surgery because lid position depends on balanced anterior and posterior lamellae tension and on the integrity of lateral canthal support. Minor laxity at the lateral canthus predisposes to involutional ectropion, and failure to respect the tarsal plate during repair can leave a rounded canthal angle or persistent scleral show. Small structures, big consequences. Teaching this view also helps clarify why full-thickness lower lid lacerations that cross the lid margin demand precise alignment of the gray line and tarsus, and why lesions arising from meibomian glands (chalazion) sit within the tarsal plate rather than in the superficial skin. Clinicians often reference the lateral canthal tendon when planning lateral canthotomy and inferior cantholysis for orbital compartment syndrome, where rapid access and correct direction of release matter.
