Medially and laterally, the outlines of the medial and lateral malleoli frame the talocrural (tibiotalar) region, with the posterior calcaneus projecting inferiorly as the heel prominence. The tendon lies superficial to the posterior aspect of the talus and calcaneus, and its distal fibers anchor to the calcaneus just superior to the plantar fat pad. Simple landmarks. Reliable. Posterior orientation matters because many ankle complaints localize to this single corridor. Achilles tendinopathy and insertional enthesopathy cluster 2 to 6 cm proximal to the calcaneal insertion, while paratenon inflammation and retrocalcaneal bursitis sit immediately deep to the distal tendon and can be aggravated by rigid heel counters or a prominent posterosuperior calcaneus (Haglund-type morphology).
