Posterior Region of the Male Wrist Dorsally

Description

Dorsally oriented anatomy of the male carpus is presented with the extensor compartments fanning distally across the dorsum of the wrist. The extensor retinaculum spans transversely over the distal radius and ulna, binding the extensor tendons as they pass from proximal forearm to the metacarpal bases. Centrally, extensor digitorum and extensor indicis tendons course toward the 2nd to 5th digits, while extensor pollicis longus angles obliquely from the ulnar side toward the thumb, crossing superficial to the extensor carpi radialis longus and brevis. Ulnarly, extensor carpi ulnaris tracks within its groove adjacent to the ulnar head. This dorsal perspective matters because the extensor retinaculum and its six fibro-osseous compartments define where tendon irritation, subluxation, and tenosynovitis localize. De Quervain disease targets the first dorsal compartment (abductor pollicis longus and extensor pollicis brevis) at the radial styloid, while intersection syndrome occurs more proximally where the first compartment crosses over extensor carpi radialis longus and brevis. Distal to Lister tubercle, the extensor pollicis longus is prone to attrition and rupture after distal radius fractures or following volar plate fixation. A small change in tendon trajectory can be diagnostic.
