Seen from the posterior aspect, the male popliteal fossa forms a shallow diamond-shaped depression centered behind the knee joint, bordered superolaterally by the biceps femoris tendon and superomedially by the semitendinosus and semimembranosus. Inferolaterally, the lateral head of gastrocnemius rises toward the femoral condyle, while the medial head of gastrocnemius defines the inferomedial margin, converging distally toward the calcaneal (Achilles) tendon complex. Superficial landmarks correspond to the deep neurovascular bundle, with the tibial nerve positioned most superficial and central, the popliteal vein deep to it, and the popliteal artery deepest against the posterior capsule. Posterior knee surface anatomy is an everyday clinical problem: this is where you palpate for a Baker cyst between semimembranosus and the medial head of gastrocnemius, and where soft-tissue swelling can mask or mimic deep venous thrombosis. The same relationships guide safe injection and aspiration around the knee, and they explain why the popliteal artery is at risk in posterior knee dislocations while the tibial nerve may be stretched by traction injuries. Clear boundaries matter. Small errors in orientation here become big errors in procedure.
