Superiorly, the cleft approaches the sacrococcygeal region, and inferiorly it narrows toward the perianal area where the rima ani (anal cleft) and anal verge would lie just anterior to the most caudal extent of the groove. Lateral to the cleft, the convexity of the buttocks reflects the underlying bulk of gluteus maximus, while the overlying skin and subcutaneous fat form the visible cluneal contours. This dorsal view matters because the intergluteal cleft is the surface corridor for common midline disease in the sacrococcygeal region, most classically pilonidal sinus and abscess. Small differences in cleft depth, hair distribution, moisture, and friction influence where midline pits and secondary openings appear, and they also shape decisions about incision placement and off-midline closure techniques that aim to reduce recurrence.
