Description
Anterior sectional anatomy through the duodenal bulb (ampulla duodeni) exposes the first part of the duodenum immediately distal to the pylorus, with the lumen expanded into the characteristic cap-shaped enlargement. Circumferential mucosa and submucosa surround the cavity, with a continuous muscularis externa forming inner circular and outer longitudinal layers that thicken proximally as the pyloric sphincter. Superior and slightly posterior to the bulb, the liver and gallbladder region would lie in close relation, while the pancreatic head sits posteromedial to the adjacent C-shaped sweep of the duodenum. Proximal meets distal quickly here. Clinically, the duodenal bulb is the classic site of peptic ulcer disease, where acid exposure and Helicobacter pylori related inflammation often produce an anterior wall ulcer with risk of free perforation into the peritoneal cavity, while posterior ulcers can erode the gastroduodenal artery and bleed briskly. That anterior perspective also reinforces why early duodenal pathology may be missed if you only think of the distal duodenum and its papillae, the bulb lacks the major duodenal papilla and has a different mucosal pattern than jejunum. Endoscopists routinely describe this segment as the duodenal cap, and it is a key landmark during upper GI endoscopy when passing beyond the pylorus.
