Description
Medially at the junction of the superior and inferior eyelids, the medial palpebral commissure (medial canthus) is presented with the lacrimal caruncle seated just lateral to the root of the nose and the lacrimal lake pooling along the medial palpebral margin. The upper and lower eyelid margins converge at the canthal angle, with the palpebral fissure extending laterally away from the medial commissure. A small crescent of conjunctival tissue and the adjacent medial bulbar conjunctiva are implied by the caruncle’s position within the medial canthal recess. Clinically, the medial canthus is where tear drainage begins, so subtle changes in this region often signal lacrimal outflow problems before the rest of the ocular surface looks abnormal. Punctal stenosis, canaliculitis, and nasolacrimal duct obstruction commonly present with epiphora and maceration near the medial commissure, and careful inspection helps distinguish these from medial canthal blepharitis or conjunctivitis. It is also the surgical neighborhood for canalicular repair after eyelid lacerations and for medial canthoplasty, where a few millimeters of misplacement can alter lid apposition and tear film distribution. Use this asset in gross anatomy and ophthalmology teaching when introducing the external landmarks of the eyelids and the lacrimal apparatus, or in clinical skills materials that cover the eye exam and assessment of epiphora. It also fits cleanly into patient-facing or professional education content on dry eye evaluation, punctal occlusion, and post-traumatic eyelid reconstruction in an adult male model.
