Seen from the lateral external aspect of an adult male ankle, the talocrural region is defined by the distal tibia and fibula embracing the trochlea of the talus to form the ankle mortise. The lateral malleolus projects distally and posteriorly relative to the medial malleolus, creating the characteristic asymmetric contour of the malleolar gutter. Anterior to the malleoli, the dorsum of the foot transitions toward the talar neck and navicular region, while posteriorly the outline rounds into the calcaneal area and the course of the Achilles tendon toward its insertion on the calcaneal tuberosity. Clear surface landmarks anchor the anatomy. That malleolar geometry matters in daily clinical work. In ankle sprain mechanisms, inversion commonly stresses the anterior talofibular ligament first, with the calcaneofibular ligament recruited as the hindfoot tilts, and the external contour around the lateral malleolus helps orient palpation when swelling obscures deeper structures. Subtle differences in lateral malleolar length and posterior position also guide reduction and fixation strategies in Weber B and C fibular fractures, where restoring the mortise width and talar tilt is the priority.
