Description
Liver parenchyma fills the frame with a diffuse steatotic change expressed on the capsular surface as a swollen, yellow-tan organ with rounded margins. The right lobe dominates laterally, with the left lobe tapering medially toward the falciform ligament region, and the normal sharp inferior border appears blunted by hepatomegaly. Subtle lobular contours of Glisson’s capsule remain visible, but the usual mahogany-brown coloration of a healthy liver is replaced by a greasy, pale yellow surface consistent with macrovesicular fat accumulation. Color carries the message. Fatty liver disease matters because gross appearance often parallels metabolic risk and guides the next diagnostic step, from laboratory evaluation (ALT, AST, GGT) to ultrasound findings of increased echogenicity and, when indicated, biopsy to stage steatohepatitis and fibrosis. In practice, clinicians differentiate bland steatosis from nonalcoholic steatohepatitis (NASH) and alcohol-associated liver disease, since persistent inflammation can progress to bridging fibrosis and cirrhosis even when early symptoms are minimal. The macroscopic emphasis on capsular color and contour supports teaching around organ-level pathology before moving to histology, including ballooning degeneration, Mallory-Denk bodies, and perisinusoidal fibrosis.
