Description
Posteriorly, the pancreatic body (corpus pancreatis) occupies the retroperitoneal space, spanning transversely from the pancreatic neck toward the tail as it approaches the splenic hilum. Its posterior surface lies anterior to the aorta and left kidney, with the splenic vein typically coursing along the posterior superior border, while the superior mesenteric vessels sit more inferior and medial near the neck. Superior relations include the celiac trunk region and the origin of the splenic artery along the superior margin, and inferiorly the pancreas approaches the transverse mesocolon and the root of the jejunal mesentery. A fixed, deep landmark. Access to the posterior pancreatic body matters in surgery and staging because this is where the gland abuts major vessels and retroperitoneal planes that determine resectability. Locally advanced pancreatic ductal adenocarcinoma often tracks posteriorly into peripancreatic soft tissue, encasing the splenic vein or approaching the superior mesenteric artery near the neck, and these relationships drive operative planning for distal pancreatectomy and lymphadenectomy. The posterior aspect also frames the path of pseudocysts and pancreatitis-related fluid collections that dissect into the lesser sac and retroperitoneum.
