Description
Centered in the female pelvis, the uterus (womb, metra, hystera) is shown as a hollow, thick-walled muscular organ with a broad fundus superiorly, a tapering body (corpus uteri) and isthmus, and the cervix projecting inferiorly toward the vaginal canal. Laterally, the uterine horns mark the transition toward the uterine tubes, while the serosal surface outlines the perimetrium overlying the myometrium and endometrium within. Supporting structures stabilizing uterine position are logically included: the broad ligament extending laterally toward the pelvic sidewall, the round ligaments coursing anterolaterally toward the inguinal canal, and the uterosacral ligaments running posteriorly to the sacrum. In the midline, the uterine cavity aligns with the cervical canal. Wall architecture matters. A thick myometrium underpins uterine contractility in labor and is the substrate for leiomyomas (fibroids), which commonly distort the uterine contour and cavity and drive menorrhagia, bulk symptoms, or infertility. Ligamentous relationships guide surgical planes: during hysterectomy or uterine artery ligation, the uterine vessels approach the cervix at the level of the cardinal (transverse cervical) ligament and cross near the ureter, the classic water under the bridge landmark. This construct also helps teach uterine version and flexion, and why loss of uterosacral support contributes to uterine prolapse.
