Description
Posterolateral soft tissues of the female lower leg are presented with emphasis on the calf (sura) contour created by the triceps surae. The medial and lateral heads of gastrocnemius sit superficially and proximally, forming the prominent bulges just inferior to the popliteal fossa, while soleus lies deep and extends more distally toward the calcaneal (Achilles) tendon. Along the lateral border, the fibular (peroneal) compartment falls anterior to the gastrocnemius margin, and the posterior midline marks the interval where the small saphenous vein and sural nerve typically course superficially toward the popliteal region. This side-oriented overview matters because most high-yield pathology in the calf is topographic: swelling and pain often raise concern for deep vein thrombosis in the posterior compartment veins, and the ability to localize tenderness relative to the gastrocnemius and soleus bellies guides the differential from muscle strain to ruptured Baker cyst tracking distally. Calf muscle tears classically occur at the gastrocnemius myotendinous junction in middle-aged recreational athletes, whereas soleus injury presents deeper and more diffuse, and both patterns differ from Achilles tendinopathy, which localizes distally and posteriorly. Palpation landmarks help. Compartment syndrome is another.
