Description

Oriented on the anterolateral neck of an adult female, the carotid triangle is bounded inferiorly by the superior belly of the omohyoid, posteriorly by the anterior border of the sternocleidomastoid, and superiorly by the posterior belly of the digastric (often with the stylohyoid alongside it). Within these borders, the carotid sheath sits deep to the platysma and investing fascia, with the common carotid artery ascending medial to the internal jugular vein and the vagus nerve positioned posteriorly between them. Superiorly, the common carotid bifurcates into internal and external carotid arteries, while the hypoglossal nerve crosses the triangle transversely and the ansa cervicalis typically lies superficial to the sheath. Surgeons and trainees return to this triangle because it frames the carotid bifurcation, carotid sinus, and proximal branches of the external carotid artery (classically the superior thyroid, lingual, and facial arteries), all of which become relevant during carotid endarterectomy and selective neck dissection. Small positional errors matter here: the internal jugular vein can obscure the artery, the hypoglossal nerve is vulnerable during high carotid exposure, and the carotid sinus can trigger bradycardia when manipulated. Palpation of the carotid pulse and bedside carotid sinus massage also depend on understanding these surface and deep relationships. A tight space.
