Seen in lateral aspect, the male foot (pes) is defined by the lateral malleolus of the fibula superiorly and the hindfoot contour formed by the calcaneus, with the talus seated between the malleoli to create the ankle (talocrural) mortise. Distal to the calcaneus, the cuboid and the lateral column metatarsals align toward the fifth metatarsal tuberosity, a palpable prominence on the lateral border. Along the plantar margin, the calcaneal tuberosity transitions into the longitudinal arch and the plantar soft tissues of the heel pad, while the dorsolateral surface slopes toward the metatarsophalangeal region of the lateral toes. Lateral foot anatomy matters because the lateral column bears load differently than the medial arch, and subtle changes in calcaneal position, cuboid alignment, and fifth metatarsal orientation track with common injury patterns and deformity. A Jones fracture at the proximal fifth metatarsal, an avulsion at the tuberosity from the fibularis brevis tendon, and peroneal tendon subluxation posterior to the lateral malleolus all localize to this side. The calcaneofibular ligament and the anterior talofibular ligament also sit in this neighborhood, anchoring the classic mechanism and exam focus of lateral ankle sprain.
