Description
Posterior surface landmarks of the female scapular region are rendered with attention to the palpable contours of the scapula (shoulder blade) against the upper back. The scapular spine runs transversely toward the lateral acromion, separating the supraspinous and infraspinous fossae, while the medial (vertebral) border parallels the thoracic spinous processes and the inferior angle points inferomedially toward the lower ribs. Lateral to the scapular body, the posterior shoulder contour reflects the deltoid and the posterior axillary fold, with the scapula sitting superficial to the posterior thoracic wall and gliding over the ribs. Surface anatomy here matters because scapular position is a proxy for both shoulder girdle mechanics and regional nerve injury. Medial border prominence and loss of smooth scapulothoracic glide raise concern for scapular winging, most classically from long thoracic nerve palsy affecting serratus anterior, and this view makes that asymmetry easy to teach and document. Clinicians also rely on these landmarks when correlating pain patterns from rotator cuff disease (supraspinatus and infraspinatus tendon pathology) and when localizing scapular fractures, which may be missed without an appreciation of the spine and inferior angle on physical examination.
