Description
Oriented in a medial perspective, the anterior (frontal) surface of the suprarenal (adrenal) gland is presented with its superior pole abutting the diaphragmatic crus and its inferior aspect directed toward the superior pole of the kidney. A smooth anterior contour contrasts with the sharper medial margin, where the gland relates to major retroperitoneal vessels depending on side, the inferior vena cava on the right and the abdominal aorta on the left. The perirenal fat and renal fascia form the immediate envelope around the gland. Small but constant topography. Surgical exposure of the adrenal often begins by understanding this anterior surface because it is the face encountered after mobilizing adjacent viscera in transabdominal adrenalectomy, and it is where the short, high flow adrenal vein must be anticipated as it courses medially to drain (right) directly into the inferior vena cava or (left) into the left renal vein. That asymmetry drives operative strategy and explains why right adrenal venous injury can rapidly obscure the field. This view also supports teaching the retroperitoneal relationships that influence imaging interpretation when an incidentaloma displaces the upper pole of the kidney or indents neighboring vascular structures on CT. Use this asset for endocrine anatomy modules, retroperitoneal dissection labs, and surgical education content discussing adrenalectomy approaches, adrenal vein sampling, or venous drainage patterns in primary aldosteronism and Cushing syndrome workups.
