Description

Presented in an anterior sectional cut through the female pelvis, the urinary bladder is opened to expose the lumen and mucosal surface, with a calcium oxalate monohydrate stone resting dependently near the bladder base. Superiorly, the dome (apex) of the bladder leads toward the urachus region, while inferiorly the bladder neck narrows into the proximal urethra. Posterior and inferior within the bladder, the trigone region is implied by the smooth mucosa between the ureteric orifices and the internal urethral orifice, and the detrusor muscle forms the thick muscular wall surrounding the cavity. Anteriorly, the bladder lies behind the pubic symphysis; posteriorly, it relates to the anterior vaginal wall and uterus/cervix in the typical adult female arrangement. Calcium oxalate monohydrate calculi are dense, often hard stones that can persist in the bladder when outflow obstruction, neurogenic bladder, or incomplete emptying promotes urinary stasis. Symptoms often include suprapubic pain, terminal dysuria, hematuria, and episodic urinary retention, with irritation greatest at the trigone and bladder neck where the stone can intermittently obstruct the internal urethral orifice. This sectional perspective also suits teaching of cystolithiasis workup and management, because it aligns well with what is targeted during cystoscopy, transurethral cystolitholapaxy, or, in select cases, suprapubic cystolithotomy. Use it in urology and pelvic anatomy teaching modules to contrast bladder stones with renal and ureteric calculi, and to orient learners to the female bladder neck and trigone during endoscopic skills training.
