Description
Seen in lateral profile, the distal sacrum continues inferiorly into the coccyx, with the sacrococcygeal joint and intercoccygeal segments aligned along the posterior midline of the lower spine. Posterior to the bony column lie the subcutaneous tissues of the rump and the superior portion of the gluteal cleft, while anterior to the coccyx the pelvic outlet soft tissues occupy the presacral and anorectal space. Inferolaterally, the inferior fibers of gluteus maximus approach the coccygeal margin, and the sacrotuberous ligament and coccygeus (ischiococcygeus) region provide recognizable posterior pelvic floor landmarks. Coccygeal anatomy matters because pain at the sacrococcygeal junction (coccydynia) is often mechanical, related to hypermobility or dislocation after a fall onto the buttocks or postpartum injury after vaginal delivery, and the lateral view is the clearest way to explain how anterior angulation places the coccyx closer to the rectum. Small differences in coccygeal curvature and segment number change palpation findings and can influence decisions about image-guided injections, manipulation, or, rarely, coccygectomy. A common pitfall is confusing coccygeal pain with pilonidal disease, which sits superficial in the natal cleft rather than at the periosteum or joint.
