Posterior surface anatomy of the female foot is presented with the calcaneal (Achilles) tendon descending in the midline to its insertion on the posterior calcaneal tuberosity, flanked by the medial and lateral malleoli superiorly at the level of the ankle. Lateral to the tendon, the contour of the lateral calcaneus and the course region of the sural nerve and small saphenous vein can be inferred along the posterolateral border toward the lateral malleolus. Medially, the heel widens toward the sustentacular region, with soft tissue bulk that transitions inferiorly into the plantar fat pad at the posterior heel. Clinical teaching often starts here because the posterior foot gives you palpable landmarks for tendon and hindfoot assessment, and the Achilles tendon is the tendon most frequently ruptured in recreational athletes and commonly thickened in tendinopathy. Pain localized just superior to the calcaneal insertion suggests insertional Achilles tendinopathy or retrocalcaneal bursitis, while a prominent posterosuperior calcaneus raises the differential of Haglund deformity with shoe-wear irritation. This angle also supports discussion of hindfoot alignment, where calcaneal varus or valgus can be appreciated relative to the midline tendon axis and contributes to overuse syndromes.
