Description

Arising along the ulnar border of the female hand, the hypothenar eminence forms the muscular pad on the palmar aspect of the little finger, framed posteriorly by the dorsal skin and extensor tendons. From a posterior (dorsal) view, the ulnar margin of the fifth metacarpal and the metacarpophalangeal region of the fifth digit sit distal to the carpal area, while the soft-tissue bulk of the hypothenar region projects on the volar side and wraps laterally toward the ulnar edge. Proximally, the hypothenar mass relates to the pisiform and the ulnar side of the flexor retinaculum at the wrist, with the base of the fifth metacarpal positioned distal and slightly radial to the pisiform. Orientation matters. This is a surface landmark more than a bony contour. Dorsal viewing of a primarily palmar structure is clinically useful because it forces clear localization at the wrist and ulnar border, where patients report pain, swelling, or paresthesia that can be misattributed to dorsal extensor pathology. Palmar trauma to the hypothenar eminence is a classic mechanism for ulnar artery injury in hypothenar hammer syndrome, and the ulnar neurovascular bundle runs just radial to the pisiform before passing into Guyon canal. That relationship drives incision planning for ulnar-sided wrist approaches and helps explain sensory symptoms in the little finger and ulnar half of the ring finger when the ulnar nerve is compressed.
