Description
Posterior cranium is centered on the occipital squama, with the external occipital protuberance (inion) marking the midline prominence and the external occipital crest descending inferiorly toward the foramen magnum. Curved superior nuchal lines extend laterally from the protuberance, while the inferior nuchal lines sit below them and frame the attachment field for the deep suboccipital musculature. Superiorly, the occipital bone meets the paired parietal bones at the lambdoid suture, and the posterolateral contour approaches the occipitomastoid region near the temporal bone. Midline landmarks are clear. These surface landmarks matter in practice because the occiput is the bony reference for posterior scalp and upper cervical approaches, from positioning and pin fixation in neurosurgery to safe incision planning in posterior fossa and suboccipital craniotomy work. Palpating the inion and tracing laterally along the superior nuchal line helps estimate the course of the greater occipital nerve as it ascends to the posterior scalp, a key consideration in occipital neuralgia evaluation and during decompression or nerve block. The foramen magnum region also anchors teaching of craniocervical junction anatomy and posterior atlanto-occipital relationships.
