Description
Centered on the midline dorsum, the vertebral column is presented from the posterior aspect, with the cervical, thoracic, lumbar, sacral, and coccygeal segments aligned in anatomical position. Spinous processes form a palpable ridge from the vertebra prominens (C7) inferiorly toward the sacrum, while paired paraspinal muscle masses (erector spinae) occupy the gutters lateral to the spinous processes. Superiorly, the scapulae overlie the upper thoracic ribs, and the medial borders of the scapulae sit lateral to the thoracic spinous processes; inferiorly, the posterior superior iliac spines and iliac crests bracket the lumbosacral junction. Posterior vertebral anatomy matters because clinicians rely on surface relationships to estimate vertebral levels without imaging, then correlate them with neurologic findings and pain patterns.
