Description
Viewed as a gross dissection of the male ankle (regio talocruralis), the distal tibia and fibula form the tibial plafond and malleolar mortise that cradle the trochlea of the talus at the tibiotalar (talocrural) joint. Medially, the medial malleolus lies inferior to the tibial metaphysis and caps the talar body; laterally, the distal fibula descends farther, creating the lateral malleolus and a deeper lateral gutter. Anterior and posterior margins of the mortise define the contours of the joint line, with the talar dome centered inferior to the tibia and bounded laterally by the fibula. Ankle mechanics are often taught as simple dorsiflexion and plantarflexion, yet the talocrural joint’s mortise-and-trochlea geometry explains why dorsiflexion is most stable, when the wider anterior talar trochlea wedges between the malleoli. That relationship matters in acute lateral ankle sprain, where inversion in plantarflexion tensions the anterior talofibular ligament first, and it also frames reduction principles for malleolar fractures and syndesmotic disruption that widen the mortise and destabilize the talus.
