The medial process of the calcaneal tuberosity lies inferiorly, continuous anteriorly with the plantar aspect of the foot, while the superior contour rises toward the posterior calcaneus. Soft tissue bulk over the medial heel pad is apparent, with the expected course of the plantar fascia originating from the calcaneal tubercle on the plantar-medial surface and running distally. Bony and soft tissue contours are presented in true medial aspect, emphasizing curvature and weight-bearing geometry. Medial heel anatomy matters because pain localization on this side often points to specific problems: medial calcaneal tubercle tenderness in plantar fasciitis, entrapment of branches of the tibial nerve in tarsal tunnel syndrome posterior and inferior to the medial malleolus, or overload of the heel fat pad with altered gait.
