Description
Centered over the posterior pelvis, the paired sacroiliac dimples (dimples of Venus) sit symmetrically just lateral to the midline at the level of the posterior superior iliac spines. Each depression overlies the posterior iliac crest region and the dorsal sacroiliac ligament complex, with the sacrum positioned medially and the iliac bones laterally. Inferiorly, the surface transitions into the superior gluteal region, and superiorly it approaches the lumbosacral junction at approximately L5 to S1. In the male body habitus, the dimples often appear shallower but remain consistent landmarks. Surface anatomy matters here because these dimples are practical proxies for PSIS palpation, a common starting point for orienting the sacroiliac joint during physical examination. Low back and buttock pain attributed to sacroiliac joint dysfunction, posterior ligament sprain, or referred pain patterns from lumbar facet joints often gets mapped in relation to this landmark, and clinicians use left-right asymmetry to discuss pelvic tilt or apparent innominate rotation in musculoskeletal assessment. A clean posterior view of this region also supports teaching of the “dimples-to-S2” relationship, since the PSIS line approximates the S2 spinous level. A small landmark. Big orientation value.
