Description
Seen from the posterior aspect, the dorsum of the male wrist is defined by the distal radius and ulna forming a shallow transverse contour just proximal to the carpal region, with the ulnar head and ulnar styloid projecting on the ulnar (medial) side. Superficial extensor tendons course distally from the posterior forearm toward the metacarpals, their cords and depressions becoming clearer with slight finger and wrist extension. Radial (lateral) to the midline, the extensor pollicis longus tendon typically angles obliquely toward the thumb, creating a recognizable change in tendon direction across the dorsal wrist. Surface landmarks dominate. This posterior wrist view matters because the extensor tendons run in tight fibro-osseous compartments beneath the extensor retinaculum, where synovial sheaths and bony ridges can predispose to focal pain syndromes and tendon dysfunction. Swelling or crepitus along the dorsal radial wrist often localizes to De Quervain tenosynovitis (first extensor compartment), while dorsal ulnar-sided prominence and tenderness may track to distal radioulnar joint irritation or extensor carpi ulnaris tendinopathy. For injections, aspiration, or dorsal portal placement in wrist arthroscopy, clinicians rely on these surface tendon lines and the palpable ulnar styloid as safe, repeatable reference points.
