Description
Centered on the dorsal midline, the posterior trunk is organized around the spinous processes from the lower cervical region through the sacrum, with the scapulae positioned posterolaterally over the upper thorax. Superficial musculature commonly visible in this female posterior view includes the trapezius spanning from the occipital region and nuchal ligament to the lateral clavicle and scapular spine, the latissimus dorsi sweeping inferolaterally toward the humerus, and the thoracolumbar fascia investing the erector spinae mass along the lumbar spine. Inferiorly, the contour of the iliac crests and the superior gluteal region frame the lumbosacral junction, a key surface landmark for level estimation. Landmarks matter here. Palpation of C7 (vertebra prominens), the medial borders and inferior angles of the scapulae, and the posterior superior iliac spines guides bedside assessment of scoliosis, scapular dyskinesis, and leg length discrepancy, and it also informs safe planning for posterior approaches in spine and paraspinal procedures. The relationship between the scapula and rib cage also underpins recognition of serratus anterior weakness (classically long thoracic nerve injury) when winging changes the resting position of the medial border.
