Description
Cut through the skin in cross-section, the epidermis forms a stratified surface overlying the dermis and hypodermis, with a dilated follicular infundibulum opening to the exterior as a conspicuous pore. Within that ostium sits an open comedo, a compact keratin and sebum plug whose superficial portion appears dark from lipid and melanin oxidation at the air interface. Deep to the follicle, the sebaceous gland lobules drain into the pilosebaceous canal, while eccrine sweat glands coil in the deeper dermis and subcutis; small arteries and veins course through the dermis alongside cutaneous nerve fibers. Open versus closed comedones matter because they are the earliest, noninflammatory lesions in acne vulgaris and they anchor the pathogenesis around follicular hyperkeratinization and altered sebum flow. A clear view of the follicular infundibulum and surface opening helps explain why closed comedones (whiteheads) remain covered by epithelium, while open comedones communicate with air and darken without implying infection or necrosis. Clear anatomy. Clinically, this is the lesion targeted by comedolytic retinoids and by mechanical extraction, and it contrasts with the deeper perifollicular inflammation and rupture that characterize papules, pustules, and nodules.
