Description
A fetus of about 37 weeks' gestation lies curled within the amniotic cavity, seen from the left side with the trunk flexed, hips and knees drawn up and the arms folded against the chest. Surface features typical of the late third trimester are visible: a rounded head with dense scalp hair, a fully modelled auricle with defined helix and antihelix, plump subcutaneous fat smoothing the limbs, and well-formed toes with nail beds on the foot. The umbilical cord leaves the anterior abdominal wall and spirals toward the disc-shaped placenta, its two umbilical arteries and single umbilical vein visible as twisted red and violet channels beneath the amnion. The placenta is shown attached to the uterine wall behind the fetus, its fetal surface traced by chorionic vessels radiating from the cord insertion. Week 37 marks the beginning of early term, when organ maturation, particularly pulmonary surfactant production and thermoregulatory fat deposition, is largely complete and the fetus normally settles into a longitudinal cephalic lie in preparation for labour. Views of this stage support discussion of fetal presentation and engagement, cord length and coiling, placental site and its relevance to praevia or abruption, and the growth assessments used to distinguish appropriate growth from macrosomia or fetal growth restriction.
