Description

Along the ulnar side of the male palm, the hypothenar eminence forms a muscular pad distal to the pisiform and proximal to the base of the little finger. Most anterior views of this region include abductor digiti minimi lying most ulnarly and superficially, flexor digiti minimi brevis positioned more radial and slightly distal, and opponens digiti minimi coursing deep along the fifth metacarpal on the palmar aspect. Palmaris brevis typically overlies the ulnar neurovascular bundle superficially, blending with the palmar aponeurosis and skin to wrinkle the hypothenar pad. Small but specific anatomy. This is the view you want when teaching the intrinsic drivers of fifth digit abduction, flexion at the metacarpophalangeal joint, and opposition with metacarpal rotation, motions that students often confuse with extrinsic flexor and extensor function. Clinically, the hypothenar compartment also anchors discussion of ulnar nerve motor supply (deep branch) and ulnar artery anatomy as it enters Guyon’s canal between the pisiform and hook of hamate, a location implicated in handlebar palsy and occupational compression neuropathies.
