Description

Centered on the dorsum of an adult male hand, the metacarpophalangeal region of the little finger lies in view with the fingers slightly abducted and the nails and periungual folds intact. A solitary exophytic, hyperkeratotic papule with a verrucous, cauliflower-like surface sits near the ulnar border at the base of the fifth digit, superficial to the extensor tendon plane and overlying the dorsal metacarpal skin. Surrounding epidermis appears unbroken, with normal dorsal skin creases and mild variation in pigmentation typical of the hand. Dorsal digital warts (verruca vulgaris) reflect human papillomavirus infection of keratinocytes, producing acanthosis and papillomatosis that create the characteristic rough surface, often with punctate thrombosed capillaries when pared. Location matters: lesions close to the MCP joint and extensor apparatus can be irritated by manual work, and their contour can mimic callus, corn, or a keratoacanthoma-like nodule when they enlarge. Treatment planning also depends on anatomic context, since cryotherapy, salicylic acid, or curettage must balance clearance against scarring, dyspigmentation, and local tendon irritation on the thin dorsal hand.
