Seen from a lateral aspect of the male anterior shoulder girdle, the deltopectoral groove (sulcus deltoideopectoralis, Morenheim’s fossa) lies between the anterior border of the deltoid laterally and the superior fibers of pectoralis major medially. Superiorly, the groove points toward the clavicle, where the deltoid and pectoralis major converge near their clavicular attachments, and inferiorly it tapers as the muscles bulk over the proximal humerus. Deep to this surface interval sits the clavipectoral fascia, spanning between clavicle and axillary fascia and separating the superficial muscular plane from the deeper neurovascular corridor. Clinically, this surface landmark guides the deltopectoral approach to the shoulder, a workhorse exposure for open reduction and internal fixation of proximal humerus fractures, shoulder arthroplasty, and access to the anterior glenohumeral joint. The cephalic vein often runs within the groove and must be identified and protected (or mobilized medially or laterally) to reduce bleeding and preserve venous outflow. A tight, palpable band in this region also helps orient injections and incisions relative to the coracoid process and underlying brachial plexus and axillary vessels.
