Description
Viewed from the left lateral aspect in an adult male, the anterior triangle of the neck is bounded superiorly by the inferior border of the mandible, medially by the anterior midline of the neck, and laterally by the anterior border of the sternocleidomastoid muscle. Within this space, the suprahyoid and infrahyoid (strap) muscles lie superficial to the laryngeal framework, with the hyoid bone spanning the triangle at the C3 level. Deep to the sternocleidomastoid, the carotid sheath occupies the lateral part of the triangle, placing the common carotid artery and internal jugular vein lateral to the larynx and pharynx, while the ansa cervicalis typically drapes on the anterior wall of the sheath. Clinically, the lateral boundary matters because it defines a working corridor for carotid endarterectomy and cervical lymph node biopsy, where dissection proceeds along the anterior border of sternocleidomastoid to reach the carotid bifurcation and adjacent nodal levels. Nerve relationships are the hazard zone: the hypoglossal nerve crosses the carotid triangle near the occipital artery, the superior laryngeal nerve descends toward the thyrohyoid membrane, and the marginal mandibular branch of the facial nerve runs at risk along the inferior edge of the mandible during high incisions.
