Description
Oriented in a lateral profile of the female neck, the carotid triangle is bounded anteriorly by the superior belly of the omohyoid, superiorly by the posterior belly of the digastric with the stylohyoid, and posteriorly by the anterior border of the sternocleidomastoid. Within this space the common carotid artery ascends deep to the investing fascia and typically bifurcates at the level of the superior border of the thyroid cartilage into the internal carotid artery (posterior and lateral) and external carotid artery (anterior and medial). The internal jugular vein lies lateral to the arteries within the carotid sheath, while the vagus nerve courses between and posterior to the vessels; the hypoglossal nerve crosses the triangle superiorly, and the ansa cervicalis often sits on the anterior wall of the sheath. Small but teachable anatomy. This lateral depiction matters because the carotid bifurcation is where clinicians auscultate for bruits, interrogate plaque on duplex ultrasound, and gain proximal and distal control during carotid endarterectomy. The carotid sinus at the origin of the internal carotid and the adjacent carotid body are key landmarks for baroreceptor reflexes and can be implicated in carotid sinus hypersensitivity, while inadvertent traction or ligation in this field risks hypoglossal palsy (tongue deviation) or vagal injury with dysphonia. The relationship between the artery, internal jugular vein, and cranial nerves is the anatomy that makes this region unforgiving.
