Arcing obliquely across the anterolateral female neck, the sternocleidomastoid (SCM) runs inferiorly from the mastoid process and lateral superior nuchal line to its two distal heads on the manubrium of the sternum (sternal head) and medial clavicle (clavicular head). A lateral perspective typically reveals the SCM as a prominent superficial strap muscle, with the posterior border forming part of the posterior cervical triangle and the anterior border contributing to the anterior triangle. Deep to its midportion lie the carotid sheath contents, while the external jugular vein commonly courses superficially across the muscle before piercing the deep fascia near the clavicle. Clinically, this is the landmark muscle for orienting neck examination and procedural planning. The spinal accessory nerve (CN XI) exits the posterior border of the SCM near the junction of its superior and middle thirds before traversing the posterior triangle to the trapezius, a vulnerable segment during lymph node biopsy and posterior triangle dissection. Palpation along the SCM also helps localize the carotid pulse just medial to its anterior border, and the muscle’s tendinous attachments at the mastoid are central when explaining torticollis patterns and the head tilt and rotation produced by unilateral SCM spasm.
