Description
Oriented in lateral profile, the female greater supraclavicular fossa is defined inferiorly by the clavicle, anteromedially by the posterior border of the sternocleidomastoid, and posterolaterally by the anterior border of the trapezius. Along this shallow depression, the skin and subcutaneous tissue drape over the deep cervical fascia, with the external jugular vein typically crossing superficial to the sternocleidomastoid on its way to the subclavian vein near the clavicle. The cutaneous supraclavicular nerves (C3 to C4) emerge at the posterior border of the sternocleidomastoid and fan inferiorly toward the clavicle and upper chest. Deep to the fossa lie the anterior scalene and middle scalene muscles, the brachial plexus roots and trunks, and the subclavian artery, while the subclavian vein runs more anterior and inferior, separated from the artery by the anterior scalene. Clinically, this region is a crowded gateway between the neck and upper limb, and the lateral perspective clarifies why needle trajectory and depth matter near the pleural dome at the thoracic inlet. Supraclavicular lymph nodes sit in the superficial fascia here and become conspicuous with metastatic disease, with left-sided enlargement (Virchow node) classically associated with abdominal malignancy via the thoracic duct. Anesthetic practice also lives in this space: the supraclavicular approach to the brachial plexus targets trunks and divisions just superior to the clavicle, where spread can also reach the phrenic nerve or sympathetic chain.
