Running along the anteromedial border of the crural region, the tibial shaft is seen in profile while the lateral compartment forms the contour of the leg. Anterior to the fibula, the extensor compartment occupies the ventral aspect, with tibialis anterior lying closest to the tibia and extensor digitorum longus positioned more laterally, its muscle belly tapering distally toward the dorsum of the foot. Extensor hallucis longus tracks between them, and the interosseous membrane spans the interval between tibia and fibula deep to these muscles. Clear compartmental anatomy. Palpating the tibial crest and tracing the extensor tendons across the anterior ankle is central to examining foot drop and anterior compartment pain, and this lateral profile helps clarify why weak dorsiflexion follows injury to the deep fibular (peroneal) nerve. Tight fascial boundaries in the anterior compartment also explain the pain with passive toe extension seen in acute compartment syndrome, where the tibialis anterior and extensor digitorum longus are early structures at risk. Surgical fasciotomy planning and safe incision placement both depend on an accurate mental map of tibia, fibula, and the intermuscular septa from this perspective. Suitable for gross anatomy and kinesiology teaching on the lower limb, and for orthopaedic or sports medicine publications covering shin splints (medial tibial stress syndrome) versus anterior tibial stress injury and exertional compartment syndrome. Radiology and physiotherapy materials can also pair this view with axial MRI or ultrasound of the anterior compartment to orient muscle position and tendon course.
