Seen from the lateral aspect, the inferior renal segment includes the lower pole cortex and medulla, with the lowest renal pyramid tapering into a papilla that projects toward the minor calyx. Lateral convexity transitions medially toward the renal sinus, where the calyceal system and segmental vessels would course, even if not fully opened in this view. Inferiorly, the fibrous capsule drapes the pole, and the corticomedullary junction outlines the boundary between renal corpuscles in the cortex and collecting ducts converging within the pyramid. Orientation matters. Teaching the kidney by segments stays clinically practical because segmental end arteries limit collateral flow, so an inferior segmental artery injury can produce a sharply demarcated lower pole infarct. Lateral exposure also mirrors what you appreciate during a posterior flank approach, where lower pole access is favored for percutaneous nephrolithotomy to reduce pleural risk, yet still demands respect for the posterior segmental arterial branches and the calyceal anatomy guiding tract placement. Lower pole calyces are a common site for retained fragments after stone treatment because dependent drainage can be less efficient.
