Description
Seen in lateral profile, the adult male hip (coxa) is centered on the femoral head seated within the acetabulum of the os coxae, with the femoral neck angling inferolaterally toward the proximal femoral shaft. Superior and lateral to the acetabular rim, the ilium forms the upper pelvis, while the pubis lies more anterior and the ischium more posterior and inferior around the acetabular cup. The greater trochanter occupies a lateral position on the proximal femur, distal to the neck, providing a palpable landmark relative to the joint line. Articular surfaces align in the coronal plane. A true lateral view matters when teaching hip biomechanics, because it clarifies anterior versus posterior acetabular coverage and the head neck offset that becomes clinically relevant in femoroacetabular impingement (cam morphology) and early chondrolabral injury. Posteriorly directed forces across a flexed hip relate directly to the mechanism of posterior hip dislocation, where the femoral head translates posterior and superior relative to the acetabulum and may endanger the sciatic nerve. Orthopedic planning also depends on this orientation, since the apparent neck shaft relationship and acetabular version influence component positioning in total hip arthroplasty and interpretation of lateral radiographs.
