Seen from the left hemithorax, the left pulmonary artery courses laterally from the pulmonary trunk toward the left lung hilum without the usual accompanying veins or bronchi. Its proximal segment lies anterior to the left main bronchus and superior to the left pulmonary veins, relationships that become clearer when the surrounding hilar structures are removed. As it approaches the hilum, the vessel aligns with the superior aspect of the root of the lung before dividing into lobar branches (not shown). Clean anatomy. Isolating the left pulmonary artery is practical when you need to teach the left-sided hilar arrangement and the point where vascular control is obtained during thoracic surgery. Surgeons often encircle and clamp this vessel during left pneumonectomy or upper lobectomy, and its proximity to the left main bronchus and recurrent laryngeal nerve territory makes dissection unforgiving when inflammation or lymphadenopathy obscures tissue planes.
