Description
Running along the medial aspect of the adult male upper arm, the medial bicipital groove (sulcus bicipitalis medialis) is defined anteriorly by the biceps brachii and posteriorly by the medial intermuscular septum and triceps brachii. Within this furrow, the brachial artery descends with its venae comitantes, accompanied by the median nerve, while the basilic vein and medial cutaneous nerve of forearm become more superficial as they approach the distal arm. Superiorly the groove relates to the axillary fold, and inferiorly it continues toward the cubital fossa, where the neurovascular bundle lies medial to the biceps tendon. Clinically, this corridor matters because it is the working neighborhood for brachial artery palpation and compression and for vascular access procedures, including brachial arterial line placement and surgical exposure during embolectomy or repair. The median nerve’s close apposition to the artery in the distal arm explains how iatrogenic injury can occur during venipuncture of the basilic vein or during medial approaches to the humerus, and why a supracondylar humeral fracture can threaten both vessel and nerve as swelling tracks into this space.
