Description

Arising from the right lung between the horizontal fissure superiorly and the oblique fissure inferiorly, the middle lobe (lobus medius) is presented from a medial aspect with its broad anterior margin directed toward the sternum. Medially, the lobe’s mediastinal surface forms part of the cardiac impression, lying adjacent to the right atrium and abutting the pericardium across the mediastinal pleura. The root of the lung sits posterior and superior to the middle lobe region, where the middle lobar bronchus and accompanying branches of the pulmonary artery and veins course toward the hilum. A medial view is the one clinicians reach for when teaching lobar anatomy in relation to the mediastinum, because fissures alone do not explain why middle lobe disease behaves differently from upper or lower lobe pathology. Middle lobe syndrome often reflects recurrent atelectasis or infection from obstruction of the middle lobar bronchus, commonly from extrinsic compression by peribronchial lymph nodes or endobronchial lesions, and the close relationship to the cardiac border explains why right middle lobe collapse can silhouette the right heart margin on chest radiographs. For procedural planning, this orientation helps clarify where a middle lobectomy dissection runs near the pericardium and hilar vessels. A tight space. Use this asset for thoracic anatomy and radiology teaching modules, case-based discussions on right middle lobe pneumonia or atelectasis, and surgical atlases illustrating right-sided lobar resection or bronchoscopy navigation to the middle lobe bronchus. It also fits medical publishing layouts that need a clean medial reference of the right middle lobe with clear fissural boundaries and cardiac adjacency. Anatomical accuracy verified by SciePro's Medical Advisory Board.
